Bus/Van Ministry Consent & Release Form

Maranatha Baptist Church of Yorktown is concerned about fulfilling the Great commission found in Matthew 28:19-20. We are reaching out to our local community to provide transportation to and from church for those who would not be able to attend these meetings due to the lack of transportation. We wish to extend this service to you and your family. However, we encourage the parents or legal guardians to attend with their child/children that ride the church vehicles. This service is not to be misunderstood as transportation to and from a daycare provider. Parents are expected to be at home to receive their child/children from the bus workers after the services are over. Also, please do not send your children in if they are running a fever or have been sick in the last 24 hours.

Bus Ministry Provides Transportation for the following services:

Sunday School @ 9:45 AM
Morning Worship @ 10:50 AM
Master Clubs (Wed) @ 6:30 PM
(when in session)

CONDITIONS OF RIDER PRIVILAGE:

Maranatha Baptist Church is concerned for your children’s safety during transportation, services, and activities. In order to accomplish our goals concerning safety and respect for others, we must have rules and guidelines to operate by, both while on the bus and while on the church grounds. Violators will receive appropriate attention in accordance to established procedure.

The following rules have been established as a guide to acceptable conduct:

1. No heads or hands out of the windows of the vehicles
2. No communication, either verbal or otherwise, out of the windows of the vehicles
3. Children must remain seated unless directed otherwise by adult supervision
4. No abusive or disrespectful language
5. No hitting, spitting or throwing objects
6. Must attend class and behave
7. Participation in organized activities is required
8. No radio, CD players, tape players, televisions, pagers, cell phones, or other audiovisual device allowed
9. No toys, electronic games, or other entertainment object or device is allowed
10. Everyone is encouraged to bring his/her Bible
11. No weapons (guns, knives, explosives of any kind, pepper spray, brass knuckles, etc.)
12. No drugs of any kind or drug paraphernalia

DISCIPLINE PROCEDURE:

Discipline will be handled as follows:

1. Offender will receive a minimum of one verbal warning (depending on the severity of the offense).
2. Parents/guardians will receive a written notice of offenses that must be immediately corrected (no additional warnings are forthcoming).
3. Parents will receive a letter describing the incident for which the child has forfeited their privilege to ride in the church vehicles.
4. MBC exercises a “ZERO TOLERANCE policy concerning items 11. and 12. in the above paragraph. Any adult or child violating either of these two rules or a repeat discipline case will forfeit their privilege to ride for a period of time. The amount of time to be determined by the bus director.

Romans 6:23 For the wages of sin is death; but the gift of God is eternal life through Jesus Christ our Lord.

II Thessalonians 1: 8 & 9 In flaming fire taking vengeance on them that know not God, and that obey not the gospel of our Lord Jesus Christ: Who shall be punished with everlasting destruction from the presence of the Lord, and from the glory of his power;

Jesus died to pay for our sin.
Romans 5: 8 But God commendeth his love toward us, in that, while we were yet sinners, Christ died for us.

Whosoever shall call on the name of the Lord in repentance shall be saved.
Romans 10: 13 For whosoever shall call upon the name of the Lord shall be saved.

Luke 13:3 I tell you, Nay: but, except ye repent, ye shall all likewise perish.

To baptize believers. We give everyone who makes a profession of faith in Christ an opportunity to be baptized as found in Acts 2: 38 & 41 Then Peter said unto them, Repent, and be baptized every one of you in the name of Jesus Christ for the remission of sins, and ye shall receive the gift of the Holy Ghost. Then they that gladly received his word were baptized: and the same day there were added unto them about three thousand souls.
(Parental/Legal Guardian permission pending).

To teach the Bible (KJV)

After a person trusts Christ as Savior, we teach him/her how to have God’s blessings on his/her life by obeying and living by the principles taught in the Bible.

I/We the undersigned understand the aforementioned belief and purpose of Maranatha Baptist Church.  I/We also understand the rules governing ridership on the church vehicles and that being unable to foresee all possible situations concerning conduct, general safety, and highway safety issues, I/We the undersigned, do hereby agree to assume all of the risks which may be encountered during the transportation to and from the services/activities and do hereby agree to hold Maranatha Baptist Church and its agents and employees, harmless from any liability, actions, causes of actions, claims, expenses, and damages on account of injury to my/our  child/children or property, even injury resulting in death, which I/We the undersigned, now have or which may arise in the future in connection with transportation of my/our child/children to and from services/activities. 


I/We expressly agree that this release, waiver, and indemnity agreement is intended to be as broad and as inclusive as permitted by the law of the State of Virginia and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.  This release contains the entire agreement between the parties hereto and the terms of this release are contractual and not mere recital. 


I/We further state that I/We have carefully read the foregoing release, waiver, and indemnity agreement and know the contents thereof and sign it as my own free act and do hereby give permission for the above named child/children to ride the Maranatha Baptist Church buses/vans. I also have received the Maranatha Baptist Church pamphlet which identifies all services and activities offered.  Any activities in addition to those identified will require a specified event permission slip.
